Statement from the Membership, Engagement and Communications, NHS Southwark Clinical Commissioning Group on the forthcoming Service User and Care Engagement Event.

Improving how we care for and support people to regain confidence and independence at home

Health and Social Care in Southwark have been looking at how they currently provide urgent and short term care and support to:

* Help people get better at home so that they don’t need to go into hospital unless they really need to.

* Settle people back into living at home if they have recently left hospital.

* Support people to remain independent, safe and well at home and prevent the need for longer term care.

They would like to hear from people and their carers who live in Southwark and their representatives who have experienced the following types of short term health and social care rehabilitation and reablement services at home:

* Intensive medical and nursing support to prevent an admission to hospital or when discharged from hospital.

* Physiotherapy and support to help people do exercises, move about and aid rehabilitation and recovery.

* Personal care to help people regain their ability to look after themselves (for example washing, bathing, meal preparation).

* Organising practical equipment to make it easier to live at home (for example a walking frame, raised toilet seat, bath board).

* Assessment of short and long term needs and information on services and activities that can support people to remain independent at home.

* Organising care and support services for people who need help in the longer term.

Usually urgent support is provided within 2 to 24 hours and for up to five days. Short term rehabilitation and reablement support is provided within two days and for up to six weeks (in some longer term cases it can be up to 12 weeks).

These services include: @home, Enhanced Rapid Response, Reablement Services, Supported Discharge Team, Early Supported Discharge for stroke clients, Double Handed Project, Community Rehabilitation and Falls Team, Community Rehab Neuro Service. 

Guy’s and St Thomas’ Foundation Trust (GSTT) and Southwark Adult Social Care will be holding an engagement event in January to share their work and thinking so far and to gain service user and carer input into shaping future changes.
